The following tables list the items required to apply for a LEOSA credential. There are currently three different types of applications that can be filed, each for a different category of candidate.

IT IS EXPRESSLY AGREED THAT IT IS NOT A GROUND FOR A REFUND OF THE FEE IF THE APPLICATION IS DENIED.


To apply for a 926B Actively Serving LEOSA credential, a fully completed, signed and notarized copy of the LEOSA Qualified Law Enforcement Application is required. This application can be downloaded HERE.

After completing the application, it should be mailed in accordance with the application instructions.

To apply for a 926C Qualified Retired LEOSA credential, a fully completed, signed and notarized copy of the LEOSA Qualified Law Enforcement Application is required. This application can be downloaded HERE.

After completing the application, it should be mailed in accordance with the application instructions.

NOTE: The 926C Retired Credential has an indefinite expiration date.

To apply for a 926C Qualified Separated Officer LEOSA credential, a fully completed, signed and notarized copy of the LEOSA Qualified Law Enforcement Application is required. This application can be downloaded HERE.

After completing the application, it should be mailed in accordance with the application instructions.

NOTE: The 926C Separated Credential has an indefinite expiration date.
